Advanced Wellness Solutions offers many different types of patient appointments.
New Patient Consultation
Prior to the first appointment we will collect primary information such as name, address, phone number, and all applicable insurance information as well as explain initial office procedures. We will also provide the necessary intake paperwork to the patient via email or direct the patient to the appropriate location on our website for new patient intake paperwork.
A new patient consultation will include:
- History of the presenting complaint
- Thorough physical exam and vitals
- Review of Findings
- Treatment Plan
- X-rays or MRI as deemed necessary. (All imaging will be done at an outside facility of the doctor’s discretion)
Established Patient Consultation
An Established patient exam will include:
- History of presenting complaint and changes from initial visit
- Full physical exam, with emphasis on previous deficiencies, and vitals
- Revised patient care plan
- Estimated number of visits or change to the number of visits needed
